a) Annual Preventive Visits/Physicals and Well-Woman Exam Visits,
b) Medicare Wellness Visits
c) Chronic Disease Management Visits:
d) New Signs/Symptoms and/or Abnormal Test Results Findings Visits, and
e) Acute Illness Visits
