Veterinary. HMDD is responsible for obtaining veterinary care for the Dog. The Fosterer may be asked to take the Dog to a veterinary appointment for a surgery, follow-up care, or other appointment(s). All possible care will be exercised in order to make this convenient for the Fosterer. If the Fosterer is unable to take the Dog to an appointment, HMDD will attempt to find transport. HMDD will provide the Fosterer with a list of who to call in the event the Dog requires emergency care. The Fosterer MUST notify a person on that list immediately. The Fosterer is not authorized to authorize any emergency treatment other than the absolute basic treatment to save the Dog’s life. Authorization for any other care must come from a member of the contact list.
Veterinary. Unless otherwise agreed, ▇▇▇▇▇’s veterinarian will be _________ and both parties will be listed on ▇▇▇▇▇’s veterinary records. Owner 1 and Owner 2 agree to have ▇▇▇▇▇ neutered by ________. Owner X will be responsible for keeping track and making routine health care and annual vaccination appointments for ▇▇▇▇▇. ▇▇▇▇▇ will attend his veterinarian yearly on ________. Whoever has ▇▇▇▇▇ in their custody on that date will ensure that an appointment is made and ▇▇▇▇▇ is examined. If a medical emergency arises with ▇▇▇▇▇, each party will inform the other immediately. The party who has ▇▇▇▇▇ in their care will may make immediate emergency life threatening decisions if they cannot reach the other party despite making reasonable efforts to do so, provided that neither party will incur a veterinary cost of more than $____ without the other party’s consent. In the event that the parties cannot agree upon any medical issue relating to ▇▇▇▇▇, they will be guided by the advice of ▇▇▇▇▇’s veterinarian. If Owner 1 and Owner 2 cannot agree on any issues related to ▇▇▇▇▇’s medical care, then Owner 2 will have the right to make such decision, provided that Owner 2 will be solely responsible for any medical costs incurred for such care that Owner 1 does not otherwise agree is appropriate.
