Validity of Claim Form. In order to be a Valid Claim Form, the Claim Form must be completed as instructed on the Claim Form with no other material alterations (excepting disputes pursuant to Paragraph 7 (C) (ix), signed, dated, and postmarked or otherwise returned to the Claims Administrator within the Claim Period.
