Transitional Care Unit Clause Samples
The Transitional Care Unit clause defines the establishment and operation of a specialized unit within a healthcare facility dedicated to patients who require short-term, intermediate care as they transition from hospital to home or another care setting. This clause typically outlines the types of services provided, such as rehabilitation, nursing care, and discharge planning, and may specify eligibility criteria for patient admission to the unit.
