(FMI) is pleased to submit this proposed scope of work for the above referenced project. This project consists of surveying and engineering design for a new 750,000 gallon elevated storage tank. Professional Engineering design services for the above mentioned project will include field survey, geotechnical study, water modeling of the City system for the design and preparation of plans and specifications for the project. The Scope of Work is further described in the following section “Design Services”. 1. Conduct an initial meeting with the City staff to further define and develop design criteria and to obtain existing information such as pertinent “as-built” plans and contract and other information available for the project. 2. Conduct a site investigation with City Staff and Project Surveyor. 3. Surveying for Design including: Establish horizontal and vertical control based on the City of FORNEY's control and monument system to establish survey control points. Perform field topographic surveys for the site. All visible improvements and trees within the project limits will be located and surveyed. Visible water, sanitary sewer, storm sewer lines, fencing and other improvements will be located and surveyed. If necessary, existing utilities uncovered by the City of ▇▇▇▇▇▇ will also be surveyed. A survey Base File for civil design will be prepared. Establish overflow elevation of existing City elevated tanks on same datum as proposed tank. 4. FMI will utilize TEAM Consultants as a subconsultant to perform a geotechnical study to develop geotechnical data for design of the new elevated tank foundation. 5. Utilizing the data a preliminary design will be made. This 35% will be submitted for review and approval by the City Staff.

Professional Engineering design services for the above above-mentioned project will include field survey, geotechnical study, water modeling of the City system for the design and preparation of plans and specifications for the project, assistance bidding the project and contract administration. The Scope of Work is further described in the following section “Design Services”.

Franchise utilities will be requested to locate their facilities so they can be included in the preliminary design. • Locate existing water meters, valves, fire hydrants for inclusion on the plans. • Perform field topographic surveys for the project site. All visible improvements and trees within with in the project limits will be located and surveyed. Franchise utilities located by 811 will be surveyed for placement on the plans. Visible water, sanitary sewer, storm sewer lines, fencing and other improvements will be located and surveyed. If necessary, existing utilities uncovered by the City of ▇▇▇▇▇▇ will also be surveyed. A survey Base File for civil design will be prepared. Establish overflow elevation of existing City elevated tanks on same datum as proposed tank.
4. FMI will utilize TEAM Consultants as a subconsultant to perform a geotechnical study to develop geotechnical data for design of the new elevated tank foundation.
5. Utilizing the data a preliminary design will be made. This 3530% design will be submitted for review and approval by the City Staff.
5. FMI will prepare an opinion of probable construction costs for the project utilizing the 30% preliminary design.

2. Conduct a site investigation with City Staff Staff, and Project Surveyor.
3. Surveying for Design including: Establish horizontal and vertical control based on the City of FORNEY's control and monument system to establish survey control points. Perform field topographic surveys for the entire length of the project to compile sufficient data for design. Shots will be taken at 50 foot intervals on a grid of the work area of the site. All visible improvements and trees within the project limits will be located and surveyed. Visible water, sanitary sewer, storm sewer lines, fencing and other improvements will be located and surveyed. If necessary, existing utilities uncovered by the City of ▇▇▇▇▇▇ will also be surveyed. A survey Base File for civil design will be prepared. Establish overflow elevation of existing City elevated tanks on same datum as proposed tank.
4. FMI will utilize TEAM Consultants as a subconsultant to perform a geotechnical study to develop geotechnical data for design of developed when the 5-acre pump station site was master planned in 2006 so no new elevated tank foundationinformation will be needed.
5. Utilizing the data a preliminary design will be made. This 3565% will be submitted for review and approval by the City Staff.
