Surgical Periodontal Services and Periodontal Maintenance. This plan covers periodontal maintenance, following documented periodontal surgery, two (2) times per plan year if at least three (3) months have passed since the completion of active periodontal surgery. This plan covers periodontal scaling and root planing once (1) per thirty -six (36) month period, per quadrant. Enhanced benefits for these services may be available for members diagnosed with diabetes and/or coronary artery disease.
