Support Planning. The Provider will offer specialist speech and language support to the Commissioners so they may contribute to the speech and language elements of any care planning documentation drawn up with either the Commissioners or through the Multi- Disciplinary Team.
Support Planning. 3.1.1.1 The Provider shall deliver a person centered Service and shall adopt such principles in the design and delivery of the Service.
3.1.1.2 The Provider shall be proactive in seeking the advice and guidance of the Council if, at any time, they are unsure of how person centered planning should be delivered within the Service.
3.1.1.3 Provider may only deliver according to what’s clearly been identified on the Personal Support Plan as being its responsibility to deliver. On no account should unauthorised support or personal care be delivered to the Service User which is outside its responsibilities in the Personal Support Plan (or within what should be regarded as a Regulated Service), unless prior written authorisation has been obtained from the Council or it is unplanned intervention which would, reasonably, put the Service User at risk should it not occur. More short term support may be delivered outside the contents of the Personal Support Plan if it aims to support the Service User through a short-term crisis or similar on the understanding that this short term support is authorised in writing by the Council.
3.1.1.4 The Provider may not deliver personal and intimate care unless its been authorised to do so in either the Personal Support Plan or by prior approval from the Council. The Provider may deliver personal and intimate care only if the Staff member delivering such care is confident, able to do so and is fully trained and delivers the support and care required in accordance with the practices and to the standards of Domiciliary Care Providers and as expressed in the National Minimum Standards for Domiciliary Care in Wales.
3.1.1.5 All work required by the Personal Support Plan must be carried out in a manner which respects the wishes and feelings of the Service User, and, if appropriate, their family.
3.1.1.6 The Provider shall operate a standardised risk assessment system which ensures that all risks presented within the Services are clearly documented, evaluated and managed;
3.1.1.7 When appropriate, the Provider shall undertake risk assessments for families and Service Users to establish their personal, social and emotional needs including any environmental factors that directly relate to and are affected by the delivery of said Services;
3.1.1.8 The Provider shall regularly check the accuracy and relevance of operational and individual risk assessments and introduce further measures or change existing measures where these are not working....
