SUPPLIER CAPABILITIES. The Contractor must be an eyeglass manufacturer, or an authorized distributor of an eyeglass manufacturer. The Contractor shall be in business for not less than three years and have experience in providing prescription eyeglasses and services to customers of similar size as outlined in the Solicitation. The Contractor must have experience with Medicaid or other similar government health care programs requiring insurance billing. The Contractor must be able to meet the needs of any participating state. The Contractor represents and warrants that neither the Contractor nor any Contractor employee or subcontractor providing services under the Contract is, during the term of this contract, sanctioned by or excluded from participation in Medicare, Medicaid, or any other federal or State health care program. The Contractor will notify DHS immediately if any such exclusion or sanction is threatened or occurs during the term of this Contract. The Contractor must be able to provide the goods and services called for in the Solicitation and be able to distribute its products on the open market through authorized, qualified eyeglass dispensers. The Contractor must have facilities, personnel, goods, materials, and a manufacturing process to demonstrate the ability to carry out effectively the specifications of a contract based upon this proposal document for the entire time the contract remains in effect.
