Summative Evaluation Record. An overall determination on performance uses baseline and follow-up scores, feedback generated throughout the year on selected indicators, general feedback generated periodically through observations and any other data or information relevant to the teacher's performance observed or gathered throughout the year. This information is captured on feedback forms and the PGP, or if applicable, the PIP. An overall rating is determined on the Summative Evaluation. Teachers will be rated as: Emerging, Developing, Proficient, or Distinguished. The Summative Evaluation should be signed by both the evaluator and the person being evaluated. The teacher's signature will indicate that a copy was received, read and discussed; it will not indicate the teacher's agreement with the contents of the report in whole or in part. If the teacher wishes to respond to the evaluation report, comment sections are provided. The Summative Evaluation must be completed by the evaluator and the teacher no later than March 15th.
