A description of (1) the process used to identify claims in the Population, and (2) the specific documentation relied upon by the IRO when performing the Quarterly Claims Review (e.g., medical records, physician orders, certificates of medical necessity, requisition forms, local medical review policies (including title and policy number), CMS program memoranda (including title and issuance number), Medicare contractor manual or bulletins (including issue and date), other policies, regulations, or directives).

Source of Data. The time period from issuance of funds to the date funds are debited from the State's account shall be determined by the appropriate clearance pattern specified in Exhibit II.
Source of Data. The time period from the date Federal funds are credited to the State's account to the date of clearance shall be determined from information captured by the Department of Administration from the agency administering the program.
Source of Data. A description of (1) the process used to identify Patient Stays and Paid Claims in the Population and (2) the specific documentation and other information sources relied upon by the IRO when performing the Rehab Review (e.g., patient medical records, Signature policies and procedures; Medicare carrier or intermediary manual or bulletins (including issue and date); practice guidelines endorsed by the American Physical Therapy Association, the American Occupational Therapy Association, and the American Speech-Language- Hearing Association; and other policies, regulations, or directives).
Source of Data. The time period from deposit to clearance shall be determined from information captured by the State Agencies' accounting system.
Source of Data. A description of the specific documentation relied upon by the IRO when performing the Cardiac Catheterization Procedures Review (e.g., medical records, physician orders, certificates of medical necessity, requisition forms, local medical review policies (including title and policy number), CMS program memoranda (including title and issuance number), Medicare carrier or intermediary manual or bulletins (including issue and date), other policies, regulations, or directives).
Source of Data. The time period from the date Federal funds are received and credited to a State's account and the date those funds are expended by check from the State's account shall be determined from information captured in the Department of Education Central Accounting System, the Highways Billing System or the State's FAMIS. The check expended (issued) date is adjusted by the check clearance pattern specified in Exhibit II to arrive at the cash disbursed date.
Source of Data. The period from when Federal funds are deposited and warrants are issued will be determined from information documented by State departments that administer the programs. The time period from warrant issuance to redemption will be determined by the appropriate program or fund redemption pattern specified in Exhibit II.
Source of Data. Central Accounting System: The time period from deposit to issuance shall be determined from information captured by XXXX (Treasury Automated Management Information System). XXXX is an electronic cash receipt system that provides bank validation dates for accounting transactions and can be used to determine when a deposit was made. AdvantageME (State accounting system) and the GQL Warehouse (Database of archived accounting transactions) can be queried to provide dates of check issuance/s in relation to the date of deposit.
