Service Utilization Summary. This summary data includes the total unduplicated number of services by age category performed by procedure code. All categories of service, as listed in the appendices of the current Medicaid Dental Services Coverage and Limitations Handbook, must be reported, including: • All dental procedures (Appendix B – D0120 through D9999) • All injectable medications (Appendix C – J0120 through J7120) • All oral and maxillofacial surgery procedures (Appendix D – 10060 through 99285)
