Service User’s information Clause Samples

Service User’s information. (a) In respect of each Claim Item you submit under this Agreement, you must include the Service User’s NHI number where this number is provided on the Prescription Form you receive or where you have already received this Service User’s NHI number previously. (b) Where you receive a NHI number on a Prescription Form that is different from the NHI you already have for that Service User, you will use the latest NHI number you have received, unless you know that NHI to be incorrect in which event you will use the correct NHI number for that Service User.
