Revocation of Request for Exclusion. Prior to entry of the Final Order and Judgment, any Settlement Class Member may seek to revoke his or her Request for Exclusion from the Class and thereby receive the benefits of the Settlement Agreement by submitting a request to the Administrator by email, mail, or through the Settlement Website (who will forward these to Co-Lead Counsel and Defendant’s Counsel) stating “I wish to revoke my request to be excluded from the Settlement Class” (or substantially similar clear and unambiguous language), and also containing the Settlement Class Member’s printed name, address, phone number, and date of birth.
