Revocation of Request for Exclusion. Any person who timely submits a Request for Exclusion may, prior to the Opt-Out Deadline, submit to the Settlement Administrator a written revocation of the Request for Exclusion, such revocation to simply bear the person’s name, address, signature, and a statement of the person revoking the prior Request for Exclusion.
