Revised Appendix. The Province may, at any time, upon consultation with the Recipient, provide any or all of the following: (a) a new Appendix 1 Permitted Purpose; (b) a new Appendix 2 Relevant PHI; (c) a new Appendix 3 Roles and Responsibilities of the Province and the Recipient; (d) a new Appendix 4 Security Requirements; and (e) a new Appendix 5 Consent Form.
