In addition to the requirements of section (a) above, Respondents are further required to submit with their Proposals the following Returnable Documents Used for Scoring and also to confirm submission of these documents by so indicating [Yes or No] in the table below: Valid proof of Respondent’s compliance to B-BBEE requirements stipulated in Section 7 of this RFP (Valid B-BBEE certificate or Sworn- Affidavit as per DTIC guidelines) Respondent’s valid proof of evidence to claim points for compliance with Specific Goals’ requirements as stipulated in Section 7 of this RFP Company Experience The service provider to submit detailed and traceable references of similar work of leasing 18 ton or higher tonnage (i.e. leasing/rental services) in the past. Proofs must be in form of testimonial letters. Transnet will verify all testimonial letters that will be provided by service providers. Testimonial letters without all 8 below aspects won't be considered: a) On accompany letterhead of the first party company which the bidding b) Type of service they got from the bidding company (18 Ton forklifts lease) c) Name of the bidding company d) Date or period of lease e) Level of satisfactory with the service provided f) Presiding officer's name g) Designation h) Signature Callout Response Time The service provider's response time for callout on breakdowns, (Only written confirmation on company letterhead will be accepted) Replacement of Broken Forklift The service provider must confirm that in the event of a breakdown they will get the equipment repaired within 5 working hours, if not, then they will replace the equipment. with the same tonnage equipment within 24-hour after the 6 hours of trying to repair has finished. The confirmation letter must be in a company letterhead.

Risk Assessments Service Provider must submit a generic risk assessment, based on the required service. Comply with Scope of Work Service Provider must submit a company letterhead confirming that they comply with the Scope of work Supporting documents – Confirming to comply with SOW on company letterhead
