RESPONSIBILITY OF MEMBERS. A. Members or applicants shall complete and submit to HMO such application or other forms or statements as HMO may reasonably request. Members represent that all information contained in such applications, forms and statements submitted to HMO incident to enrollment under this Certificate or the administration herein shall be true, correct, and complete to the best of the Member’s knowledge and belief.
