Common use of Requisition Documentation Clause in Contracts

Requisition Documentation. During a disaster, the recipient healthcare facility will accept and honor the donor healthcare facility’s standard (HICS) requisition forms (see appendix). Documentation should detail the items involved in the transaction, condition of the material prior to the loan (if applicable), and the party responsible for the materials. All requests given through electronic means, such as WebEOC shall be considered valid requests.
