– Request and Verification. The employee shall submit a written request for salary continuance, accompanied by a written medical statement verifying the illness, injury or other disability and the expected length of absence. In the event that an employee is physically or mentally unable to submit the necessary paperwork, the employee’s spouse, partner or legal representative can make the election on behalf of the employee to use salary continuation and his or her decision accepted.
