REPORTS OF PHP CHANGES. The governing body or the administrator of the PHP shall submit in writing to DHA any proposed significant changes within the PHP no later than 30 days prior to the actual date of change; failure to report such changes may lead to termination of this agreement. A report shall be made concerning the following items: (a) Any change in administrator or primary professional staff. (b) Any change in purpose, philosophy or any addition or deletion of services or programs. This includes capacity or hours of operation.
