Common use of Reporting Unsuccessful Security Incidents Clause in Contracts

Reporting Unsuccessful Security Incidents. The Participant shall annually provide a report to HIO describing in summary form the nature and extent of Unsuccessful Security Incidents concerning Patient Data or the Participant’s access or use of the System or the Services experienced by the Participant during the period covered by that report, as more specifically described in the Policies and Procedures.

Reporting Unsuccessful Security Incidents. The Participant shall annually provide a report to HIO describing in summary form the number, nature and extent of Unsuccessful Security Incidents concerning Patient Data or the Participant’s access or use of the System or the Services experienced by the Participant during the period covered by that report, as more specifically described in the Policies and Procedures.

The Participant shall annually provide a report to HIO describing in summary form the nature and extent of Unsuccessful Security Incidents directly related to the HIO that were concerning Patient Data or the Participant’s access or use of the System or the Services experienced by the Participant during the period covered by that report, as more specifically described in the Policies and Procedures.

The Participant shall annually provide a provide, as required by the Policies, Procedures and Standards, an annual report to HIO describing in summary form which summarizes the nature and extent of any and all Unsuccessful Security Incidents concerning involving Patient Data or the Participant’s access or use of the System or the Services experienced by the Participant that occurred during the period covered by that such report, as more specifically described in the Policies and Procedures.

The Participant shall annually provide a report to HIO describing in summary form the nature and extent of Unsuccessful Security Incidents concerning directly related to the HIO that wereconcerning Patient Data or the Participant’s access or use of the System or the Services experienced by the Participant during the period covered by that report, as more specifically described in the Policies and Procedures.
