The Reporting Documentation clause requires parties to provide specific records or evidence related to their activities or obligations under the agreement. Reporting Documentation. All documentation relating to complaints and appeals will be kept strictly confidential and will not be accessible to anyone who is not directly involved in handling the situation. Any material about the outcome of the complaint and appeal will be placed on the appropriate client/student and/or personnel file and will only be accessible to the authorised staff of ▇▇▇▇▇ Group and the individual concerned. All outcomes and recommendations will be provided to the client/student in writing within 14 days of being made.

(2) The hourly wage of each job created with separate bands of wages;
(3) The sum of the hourly wages and cost of benefits;
(4) The date the job and wage goals will be reached;
(5) A statement of goals identified in the subsidy agreement and an update on achievement of those goals;
(6) The location of the recipient prior to receiving the business subsidy;
(7) If the recipient was previously located in another site in Minnesota, the reason that the project was completed on this site;
(8) If the company has a parent corporation, the name and address;
(9) A list of all financial assistance by all grantors for the project; and
(10) Other information the City may request.

The Contractor shall collect and maintain documentation verifying that the 5-18 year old school-age children being served are refugees. These records will be maintained and accessible to ▇▇▇▇ as deemed necessary to enable ▇▇▇▇ to perform its duties and assure the Contractor’s compliance with funding regulations and conditions for the contract.
Reporting Documentation. The Contractor shall submit reports/documentation in the form and content as specified in section IV. Payment above. The Contractor shall collect, maintain, and submit acceptable documentation verifying that the children being served are eligible for refugee services per current Office of Refugee Resettlement guidelines and policies. In general, documentation must include information on country of origin, date of arrival, date of birth, and A# (paper Form I-94/Transportation Letter), Passport Number (electronic I-94), Registration # (Visa), or USCIS# (Permanent Resident Card). Acceptable documentation is described in ▇▇▇ State Letters 00-17 and 09-17, available at ▇▇▇▇://▇▇▇.▇▇▇.▇▇▇.▇▇▇/programs/▇▇▇/policy/sl00-17.htm; and ▇▇▇▇://▇▇▇.▇▇▇.▇▇▇.▇▇▇/programs/▇▇▇/policy/sl09-17.htm. If serving full-time high school students over the age of 18, the Contractor shall submit additional documentation as required by the federal Office of Refugee Resettlement. In addition, the Contractor shall submit backup documentation verifying the number of refugee parents and district/community staff served under this contract. These records will be submitted to, and accessible to ▇▇▇▇ and DSHS/Office of Refugee & Immigrant Assistance (DSHS/ORIA) as deemed necessary to enable ▇▇▇▇ and DSHS/ORIA to perform its duties and to assure the Contractor’s compliance with funding regulations and conditions for the contract.
Reporting Documentation. ● Service Ticket for every visit. ● After‑Action Report after each urgent call or entrapment, detailing root cause, metrics, and corrective plan. ● Maintain log in machine room; send electronic copy with monthly invoice.
