Common use of Reimbursement for Attested Time Clause in Contracts

Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to four (4) hours at Twenty Five Dollars and Zero Cents ($25.00) per hour rounded to the nearest whole hour. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Incident and the time associated with each action. Claims for Attested Time are capped at One Hundred Dollars and Zero Cents ($100.00) per individual.

Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to four ten (410) hours at Twenty Five Dollars and Zero Cents forty dollars ($25.0040) per hour rounded to the nearest whole hour. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Security Incident and the time associated with each action. Claims for Attested Time are capped at One Hundred Dollars and Zero Cents ($100.00) 400 per individual.

All Settlement Class Members may submit a claim for reimbursement of Attested Time up to four (4) hours at Twenty Five Dollars and Zero Cents ($25.00) 20.00 per hour rounded for self-certified Attested Time that is fairly traceable to the nearest whole hourSecurity Incident. Settlement Class Members can receive reimbursement of may submit an additional Attested Time with a brief description of the actions taken in response request for up to eight (8) additional hours at $20.00 per hour for documented time that is fairly traceable to the Data Security Incident and the time associated with each action. Claims for Attested Time are capped at One Hundred Dollars and Zero Cents ($100.00) per individualsupported by Reasonable Documentation.

To make a Claim, Settlement Class Members can receive reimbursement must complete the appropriate sections of Attested Time the Claim Form, with a brief description of the actions taken in response to the Data Incident Breach and the time associated with each action. Claims for Attested Time are capped at One Hundred Dollars spent, and Zero Cents ($100.00) per individualattest under penalty of perjury that the information provided is true and correct.
