Records and Facilities. Provider agrees:
2.1. To give the Department of Health and Human Services (HHS), and the General Accounting Office (GAO) or their designees the right to audit, evaluate, inspect books, contracts, medical records, patient care documentation, other records of subcontractors, or related entities for the later of six (6) years, or for periods exceeding six (6) years, for reasons specified in the federal regulation.
2.2. To safeguard the privacy of any information that identifies a particular Member and to maintain such records in an accurate and timely manner. *** Confidential Information omitted and filed separately with the Securities and Exchange Commission.
Records and Facilities. The Company's chief executive office and chief place of business and the office where the Company keeps its records concerning its accounts, contract rights, chattel paper, instruments, general intangibles and other obligations arising out of or in connection with the sale or lease of goods or the rendering of services or otherwise ("Receivables"), and all originals of all leases and other chattel paper which evidence Receivables, are located in the State of Indiana, County of ▇▇▇▇▇▇▇, at ▇▇▇▇ ▇▇▇▇▇▇▇▇▇ ▇▇▇▇▇, ▇▇▇▇▇▇▇▇, ▇▇ ▇▇▇▇▇. The Company will provide the Agent with prior written notice of any proposed change in the location of its chief executive office and will not change the location of its chief executive office without providing at least 20 days' prior written notice to the Agent. The Company's only other offices and facilities are at the locations set forth on Schedule 1(b) hereto. The Company will provide the Agent with prior written notice of any change in the locations of its other offices and the facilities. The tax identification number of the Company is ▇▇-▇▇▇▇▇▇▇. The name of the Company is Casting Technology Company, and the Company operates under no other name. The Company shall not change its name without providing at least 20 days' prior written notice to the Agent.

2.1. To give the Department of Health and Human Services (HHS), CMS and the Comptroller General or their designees the right to audit, evaluate, and inspect any books, contracts, computer or other electronic systems, medical records, patient care documentation, and other records of Provider, its contractors, subcontractors, or related entities for the later of ten (10) years, or for periods exceeding ten (10) years, for reasons specified in the federal regulation. 2.2. To safeguard the privacy and confidentiality of any information that identifies a particular Member, and abide by all Federal and State laws regarding confidentiality and disclosure of medical records, or other health and enrollment information. [422.118(a)]
2.3. To maintain the records and information of Members in an accurate and timely manner. [422.118(c)]
2.4. To ensure that medical information pertaining to Members is released only in accordance with applicable Federal or State law, or pursuant to court orders or subpoenas. [422.118(b)]
2.5. To comply with ▇▇▇’s standards for timeliness for appointments and waiting times for each type of service. [422.112(a)(6)(i)]
2.6. To ensure timely access by Members to the records and information that pertain to them. [422.118(d)]
