Recertification. Upon expiration of the time period which the health care provider originally estimated that the employee needed for his/her own serious health condition, the City may require the employee to obtain recertification if additional leave is requested.

Recertification. An employee who has taken leave because of a serious health condition or that of a family member is required by the Board to obtain subsequent written recertification of the medical condition every five (5) weeks during the duration of the condition. The Board also requires an employee, or his/her representative, on leave under this provision to report periodically, in writing, at least every month on his/her status and the intention of the employee to return to work.
Recertification. If, during the term of this Agreement, a group or groups of employees covered by this Agreement are required to meet the competency requirements of the Health Practitioners Competence Assurance (HPCA) Act 2003, the employer will accept responsibility for providing the necessary resources and costs to meet these requirements. If enrolment in a recognised Continuing Professional Development (CPD) points programme forms part of the HPCA competency requirements, the employer will reimburse the enrolment fees.

Recertification. Requests for maintenance on Products purchased from sources other than an F5 VAR or directly from F5 (i.e. used or purchased from an online auction), or where maintenance has lapsed on the Product for more than 180 days, will first be subject to an inspection by a representative of F5 at the rate of $10,000 USD per unit ($20,000 USD for redundant systems) payable to F5 Networks. The inspection will determine if the unit is at a maintainable state and eligible for coverage. Once the unit has passed inspection, a F5 support services and additional services may be purchased at the current published rates.

Recertification. The County shall pay State of Florida recertification fees for full time Fire Safety Inspector (including Battalion Chiefs), Paramedic and EMT recertification and shall also provide reimbursement for employees who take required recertification courses, if such course is not offered by the County. Any recertification course required by the State of Florida, and which is not available to the employee while on duty, will be taken by the employee while off duty and shall not be considered as time worked. Any recertification course required by the County, but which is not required by the State of Florida, and not available to the employee while on duty, will be taken by the employee and shall be considered as time worked. The County will continue to reimburse for fees and courses relating to certifications and recertifications that have been previously paid by the Department and any additional courses/training determined to be beneficial to the Department and approved by the Fire Rescue Administrator.
Recertification. In instances where the minimum duration of leave anticipated by the original certification is more than 30 days, Humboldt County may require the employee to recertify that the original medical condition still exists. Such requests can be made no more frequently than the minimum duration of the leave requested (e.g., 40 days) or once every six months in connection with an absence. In situations in which the minimum duration of leave anticipated by the original certification is less than 30 days, Humboldt County may request recertification if the employee requests an extension of leave, the circumstances described by the original certification have changed significantly, or Humboldt County receives information casting doubt upon the continuing validity of the certification. Recertifications are not permitted for leave to care for a covered service member.
Recertification. An employer may request recertification at reasonable intervals, but not more often than every 30 days. The employer may request more often, however, if the employee requests an extension, circumstances change, or the validity of an initial certification is questioned.
