QUALIFICATIONS QUESTIONNAIRE. This form must be completed and signed by an authorized person representing the bidder and the form must be notarized. Bidder must include all certifications and other evidence that the bidder and its service technicians meet the threshold requirements necessary to perform the services required hereunder. Be sure to attach all relevant evidence of training, prior experience and certifications for service technicians who will be performing services. Must be completed and signed by Bidder. QUALIFICATIONS QUESTIONNAIRE. Each bid must be accompanied by a completed "Qualifications Questionnaire", including required attachments, concerning both the bidder's experience as well as the bidder's financial status. All information must be complete, accurate and in the form requested. The Qualifications Questionnaire may be found in the "Bid Form B” of this document.
