Provider Listing. Provider acknowledges and agrees that the CMO shall be entitled to use (i) the name(s), business address(es), and phone number(s) of Provider and (ii) in addition to the foregoing, information about education, specialty, subspecialty, licensure, certification, hospital affiliation, office hours, languages spoken, and any other demographic information for any individual Participating Provider employed by or under contract with Provider to provide services under this addendum, for the purposes of enrolling and referring Members, marketing, complying with Care Select Program and CMO program requirements, reporting, and otherwise carrying out the terms and conditions of this addendum.
