The Program Leadership clause defines the roles, responsibilities, and authority of individuals or committees tasked with overseeing the management and execution of a specific program. Typically, this clause outlines who will serve as program leaders, their decision-making powers, and their obligations to report on progress or issues. For example, it may specify that a steering committee will meet regularly to review milestones and address challenges. Program Leadership. PCPs have a defined steering group and medical champion for the practice’s behavioral health integration activities. Patients are routinely screened prior to or during annual physical exams with a standardized tool for both depression and alcohol use. PCP staff providing direct service are trained in patient activation and health behavior change techniques. PCPs and (other members of the care team as appropriate) deliver evidence‑based interventions in consultation with behavioral health providers. PCP practices have available and regularly use referral information for self‑help groups, and offers books, pamphlets and websites that ▇▇▇▇▇▇ patient self‑help PCPs provide linkages that facilitate the connection of patients with community resources such as gyms, churches, housing and food support.
Program Leadership. Employer shall provide a Director of the Program, who is an active member of the Hospital’s Medical Staff, and who oversees the clinical and didactic aspects of the Program. Additionally, Employer shall comply with the accreditation and institutional requirements in accordance with the ACGME as well as the other responsibilities as described in the Program Manual(s).
