Premium Continuance Sample Clauses

Premium Continuance. Whenever an employee has been granted an approved leave of absence without pay, as a result of personal illness, injury or disability, that prohibits the employee from performing their assigned duties, the City agrees to continue the health insurance premiums for the employee only, for a period not to exceed three months. The City Manager may, at his discretion, extend the continuance of paid health premiums (employees only) for an additional three months for an employee who suffers a major disability. A major disability is defined as an illness or injury, such as heart disease or cancer, which requires a long-term rehabilitation period.
