Policy Application. Questionnaire provided by BEE INSURANCE CORP, which contains a set of questions regarding the identification of the INSURED and the Beneficiary(ies). It also contains questions on a detailed description of the background of the Proposed INSURED's family ascendants, of his/her own past and present state of health, a formal statement of his/her state of health at the time of contracting and other data of his/her current profession, activity or trade that may influence the estimate of the service. All questions must be fully and accurately answered by the Proposed INSURED and such statement shall constitute the legal basis for the issuance of the requested Life Contract. Also, the application contains the details of the coverages that are intended to be additionally contracted, distinguishing the basic coverage from the optional ones, expressly stating that the latter shall not be mandatory for the INSURED to contract. The Proposed INSURED, when filling out the application, must accurately declare to BEE INSURANCE CORP, in accordance with the questionnaire and other requirements indicated by BEE INSURANCE CORP, all circumstances known to them that may influence the valuation of the service.
