Plan Coverage. The City will provide a medical/dental, vision, and prescription drug insurance plan for all eligible employees including all bargaining unit members and their eligible dependents.
Plan Coverage. Life insurance is provided through a $20,000 term policy on the employee only, together with a $10,000 term policy on an employee's spouse and $5,000 term policy on the employee's dependents. Employees have the option to purchase family coverage at the negotiated benefit rate which provides a $10,000 term policy on an employee’s spouse and/or $5,000 term policy on the employee’s eligible dependents.

Plan Coverage. Smart Houzz Systems Service Plans include equipment warranty coverage and/or equipment maintenance for one year starting 30 days after this agreement is signed or from date you requested except as otherwise stated in the plan you selected. Coverage is for owned or rented residential property, not commercial property or premises converted into a business. You acknowledge receipt of a copy of this Agreement which outlines your equipment coverage under the Plan.
Plan Coverage. A Nurse on pregnancy, parental, or adoption leave may continue to be covered under the Employer's group life insurance and medical plans, provided she pays her portion of the premium each month of her leave.
Plan Coverage. The Employer will endeavour to maintain the current level of benefits coverage comparable to its current plan as at June 23, 2017, however, the Employer, in consultation with the Union, reserves the right to change its benefits programs, including but not limited to, adding, reducing or altering coverage, changing benefits carriers, amending plan provisions and/or merging plans as may be necessary.
Plan Coverage. All coverage will remain equal to or greater than those currently in effect for the life of this Agreement. The Board has the right to change carriers but in no case may the Board reduce benefits or services unless recommended by the CWEA and the Board’s Issues Council and approved by the CWEA’s and the Board’s negotiation committees. Two members of OAPSE leadership will be included in the meetings with CWEA and the Board’s Issues Council regarding any coverage changes. The Insurance Committee will continue to work on insurance plan design changes and will make a recommendation to the Issues Council during the 2023-2024 school year to be ready for negotiations for a successor agreement.
Plan Coverage. Coverage under these plans will be maintained only as long as the University can provide it. Coverage and eligibility shall be governed by the terms of the University Plan and the Society will make available copies of the Agreement to the Union. If and when coverage cannot be provided by the University for the Society, the Society will enter into negotiations in an attempt to endeavour to replace the existing benefit plan with one comparable and at existing rates.
Plan Coverage. From and after August 31, 2006 and until the earlier of May 31, 2009 or such time as another employer makes available to Pine medical and dental coverage comparable to that which the Company currently provides to Pine, the Company shall provide, under COBRA during the last 18 months of the Term and at its expense during all of the Term, medical and dental coverage for Pine under, and subject to the terms and conditions of, such group insurance plans as the Company now and in the future makes available generally for its employees. Nothing in this Section 3 shall be construed to require the Company to institute or maintain any or any particular benefit plan, program or policy. If and to the extent that this Section 3 conflicts with any COBRA notice or other document issued by the Company at any time, the provisions of this Section 3 shall prevail.
Plan Coverage. The School Board will provide all full-time unit employees with single, two-person or family health insurance with coverage through the New Hampshire Municipal Association, with the following plans being offered effective July 1, 2006: Blue Cross Comp 100 (Mail In Prescription $1) Blue Choice 2-Tier (Retail Prescription $3/$15; Mail In Prescription $1) Xxxxxxx Xxxxxxxx Blue (Retail Prescription $3/$15; Mail in Prescription $1)
