Physician’s Services. a. Consultation, examination and treatment by a Plan Physician at a Plan Physician’s Office. See attached provider network for participating providers. b. Periodic health assessment, to include adult health examinations, medical history, physical examination, necessary laboratory, x- ray and other screening or diagnostic tests as indicated by the age, sex, medical history, or physical examination of the Covered Person when deemed beneficial and provided or ordered by a Plan Physician in accordance with the Plan’s standard protocols. c.
