Physical capacity to. (5. N/A for Psychology students) • use technical equipment, which includes having the dexterity to undertake clinical procedures and handle, maintain and program equipment; • apply clinical procedures (e.g. physical examination, wound management), support patients/clients and perform cardiopulmonary resuscitation (CPR); and • manage essential equipment and materials. If you answer YES to any of the questions in the Safety in Practice Disclosure, you are required to have the CHM Health Assessment Form completed by a Medical Practitioner. Declaring a medical, physical and/or psychological condition will not automatically exclude you from undertaking PEP. The University is committed to anti-discrimination practices and will provide reasonable adjustments to enable students to participate in PEPs as long as safety requirements are not compromised. Your health information will only be used for the direct purpose for which it is collected. The information provided by you in Section 9 may be disclosed to the healthcare provider at which you are undertaking your PEP, in which case that provider will be informed that they are bound by the privacy provisions of the University and that they are required to contact you in relation to the use, storage and disclosure of your health information. If you do not provide the information requested you may be refused PEP.
