Page Overview. The Child Placement Agreement page is created via the Create Case Work pop-up page. The Create Case Work pop-up page can easily be accessed from the Create Case Work Command Button on the FSFN Banner Bar or by means of the Create drop down on the FSFN Banner Bar. Another way to access the Create Case Work pop-up page in order to create the Child Placement Agreement page is through Case Book. Once navigated to the Case Book page, the Child Placement Agreement hyperlink can be found in the Participants Actions panel on the right hand side of the page. Clicking the hyperlink will launch Create Case Work page that will be defaulted to Child Placement Agreement value and the Case Name selected. Note that if the participant for whom the Child Placement Agreement page is being created is 18 years of age or older, FSFN will provide a validation message asking the user if he/she is sure they wish to create a Child Placement Agreement page for a participant that is 18 years of age or older. In addition, a participant can only have one active Child Placement Agreement at a given point in time. This is regardless of the Purpose. Therefore, if a participant has an “Active” (non-terminated) Child Placement Agreement, FSFN will provide a validation message: “The selected participant has an “Active” Child Placement Agreement in Case ID < >. Please access the existing Child Placement Agreement to make the appropriate changes.” The same validation message will appear if Create Case Work is launched from the FSFN Banner Bar or if Create is selected from the Menu or Case Book. The Child Placement Agreement has an associated Resource labeled Child Placement Agreement which has the following associated Actions: Create; Edit; Update; and View. There are two Security Profiles, one labeled Child Placement Agreement – All and the other Child Placement Agreement – View, permit users to access the page in either edit or view mode. Users with the Security Profile labeled Child Placement Agreement - All will have the following Actions selected: Create; Edit; Insert; Update; View; and Open the Child Placement Agreement template. Users with the Security Profile Child Placement Agreement – View will have the following Actions selected: View and Open the Child Placement Agreement template.  “Child Placement Agreement - All" will be assigned to the following Security User Groups: CPI - FSDMM; CPI Supervisor - Spec FSDMM; CPI Specialist L2 Enhanced - FSDMM; Child Case Manager Support - FSDMM; Child Case Supervisor Specialist - FSDMM; Provider Specialist; Provider Supervisor; DCF Licensing Specialist; and SuperUser.  "Child Placement Agreement - View" will be assigned to all remaining Security User Groups.

Page Overview. The Existing Child Placement Agreement Pages can be accessed via the associated Case Book and/ or Person Book, center drop down panel. Upon selecting Child Placement Agreement, the center panel will display all existing Child Placement Agreement pages that were created and/or last updated within the last 24 months. Each Child Placement Agreement page is created via displays as: Child Placement Agreement - <Type> as the Create hyperlink; Last Name, First Name Middle Name as plain black text directly beneath the hyperlink as row 2 (NOTE: ; then Most Recent Effective Date and Terminate Date display in plain black text as row 3. Most Recent Effective Date and Terminate Date are field labels which will display at all times. Each hyperlink will launch the associated Child Placement Agreement Page in edit or view mode based on where the Case Work pop-up pageBook page was accessed from in combination with the users’ security. The Create 2 differences from Case Work pop-up Book to Person Book are 1) Person Book displays the associated Case ID for each Child Placement Agreement page can easily be accessed from in parenthesis, within the Create Case Work Command Button hyperlink, directly to the right of the Type; and 2) because Person Book is specific to one participant, if there is a current Child Placement Agreement (not terminated) it will display first and then all terminated Child Placement Agreement pages will display newest to oldest based on the FSFN Banner Bar or by means of the Create drop down very first Effective Date documented on the FSFN Banner Barpage. Another way of accessing existing Child Placement Agreement Page(s) is to access navigate on the Create Desktop to the Case Work pop-up page in order to create Folder icon and locate the Child Placement Agreement icon. All existing Child Placement Agreement pages will be displayed as hyperlinks that will launch the associated Child Placement Agreement page is through Case Bookin edit or view mode based on where the outliner was accessed from in combination with the users’ security. Once navigated This applies to the Desktop Outliner (Case Book and Participant View) and Search Outliner (Case tab – Case or Participant View, Person tab, Provider tab, and Worker tab). In accessing all existing Child Placement Agreement pages, they will display newest to oldest based on the very first Effective Date documented on the page. If more than one child has the same Effective Date, they will display based on Last Name, First Name, and Middle Name alphabetically. Finally, if a current, non-terminated Child Placement Agreement Page exists, it can be accessed through a child’s Out of Home Placement page (if one also exists), as well as the Person Provider page if the child is in an Out of Home Placement and is placed with a Person Provider (ie. ▇▇▇▇▇▇ Care, Pre-Adoptive Placement, etc.). Upon successfully saving the Out of Home Placement page for the first time, a hyperlink displays in the header group box that indicates ‘Behavior Management Plan’ or ‘Care Precautions’ based on the Type of Child Placement Agreement the child currently has. Based on how the Out of Home Placement page was accessed determines how the Child Placement Agreement Page launches and is based on the current user’s security. 4 Child Placement Agreement Page 4.1.1 Page Overview Upon initial creation of the Child Placement Agreement page, the page will automatically open and default to the second tab. The second tab, Child Placement Agreement Details, allows users to document the child placement agreement details. Upon subsequent launchings of the page, the Child Placement Agreement hyperlink can be found in page will default to the Participants Actions panel on the right hand side of the page. Users Age displays as a numeric value; plain black text; read only; displays the age calculated based on the date of birth currently documented on the child’s Person Management record compared to the current system date; each time the page is launched or saved, the Age will update based on the Date of Birth that is currently captured on Person Management compared to the current system date. Once the page is terminated, however, the Age is frozen and no longer updates. DOB Date of birth associated with the Security Profile Child Placement Agreement – View will have the following Actions selected: View and Open child for whom the Child Placement Agreement templatepage was created.  “Date of Birth displays as MM/DD/YYYY; plain black text; read only; displays the date of birth currently documented on the child’s Person Management record; if no Date of Birth is documented, the label will still be displayed; each time the page is launched or saved, the Date of Birth will update based on the DOB currently captured on Person Management. Once the page is terminated, however, the Date of Birth is frozen and no longer updates. Current Placement Current placement associated with the child for whom the Child Placement Agreement - All" will be assigned to page was created. Current Placement displays the following Security User Groups: CPI - FSDMM; CPI Supervisor - Spec FSDMM; CPI Specialist L2 Enhanced - FSDMM; Child Case Manager Support - FSDMM; Child Case Supervisor Specialist - FSDMM; Provider Specialist; Provider Supervisor; DCF Licensing Specialist; current Out of Home Placement documented for the child (Service Category – Service Type) each time the page is launched or saved until the page is terminated, at which time the Current Placement becomes frozen and SuperUserno longer updates.  "Current Placement displays as a hyperlink which launches the associated OHP in Edit or View mode based on from where the Child Placement Agreement - View" was accessed in combination with the users security; current means the Out of Home Placement has been fully approved and is not ended; conversely, if the Out of Home Placement has information documented on the Removal/ Placement Ending pop-up page, but it hasn’t been fully approved, the placement is still current and hyperlink will display. Case Name Case Name displays as the Case Name documented on Maintain Case page. Case Name displays as a hyperlink to launch the Maintain Case page in Edit or View mode based on existing Security and from where the Child Placement Agreement page was accessed; displays as Last Name, First name, Middle Name with the associated Case ID in parenthesis; updates each time the page is launched or saved until the page is terminated at which time the Case Name becomes frozen and no longer updates. Status Status displays the Status of the Child Placement Agreement page which is either Active or Terminated. Upon initial creation prior to saving for the first time, the Status will be assigned blank (null); once the page is saved for the first time it will update to all remaining Security User Groupsdisplay as Active; once the Terminate fields are documented and successfully saved, the Status will automatically update to display as Terminated. NOTE: there is not a ‘pending’ status for this page.
