Common use of Outcome Goals Clause in Contracts

Outcome Goals.  Establish and maintain a case load of 15-25 individuals/families from the “By Name List” described above, depending on Vulnerability Index – Service Prioritization Decision Assistance Tool  85% of individuals/families served will have secured permanent housing.  50% of individuals and families served will have secured a stable income source - Employment, Social Security Benefits, etc.  50% of individuals and families served will have established a primary health care provider.
