Failure to submit all forms below according to the Offer Form Instructions may result in the submitted proposal being determined to be non-responsive. • Signed Offer and Acceptance Form • Completed Attachment I or separate narrative prepared by Offeror – Questionnaire located on Page 44 of RFP. • Completed Attachment II – Pricing Instructions document located in attachment tab of ProcureAZ • Completed Attachment – Key Personnel document located in attachment tab of ProcureAZ • Completed Attachment – Subcontractors document located in attachment tab of ProcureAZ • Completed Attachment – Organizational Profile document in attachment tab of ProcureAZ
