Notice to Covered Persons. If you are covered by more than one health benefit plan, and you do not know which is your primary plan, you or your Provider should contact any one of the health plans to verify which plan is primary. The health plan you contact is responsible for working with the other plan to determine which is primary and will let you know within thirty calendar days. Questions about Coordination of Benefits? Contact the Washington State Office of the Insurance Commissioner at 0-000-000-0000.‌ SUBROGATION AND REIMBURSEMENT If we make Claims payment on your behalf for Injury or Illness for which another party is liable, or for which an uninsured or underinsured motorist (“UIM”) or personal injury protection (“PIP”) insurance exists, we will be subrogated to any rights that you may have to recover compensation or damages from that liable party related to the Injury or Illness, and will be entitled to be repaid, for payments we made on your behalf, out of any recovery that you obtain from that liable party after you have been fully compensated for your loss. The liable party is also known as the "third party" because it is a party other than you or us. A third party includes a UIM carrier, because the UIM carrier stands in the shoes of the third party tortfeasor and because we exclude coverage for such benefits.
