The CCG has a number of clinical priorities which relates to the care for our patient population in nursing homes within the Ipswich and East Suffolk area as follows:  To improve the health of those most in need  To improve access to mental health  To improve outcomes for patients with diabetes to above national averages  To improve care for frail elderly individuals  To allow patients to die with dignity and compassion and choose their place of death The remit of this contract is to improve the clinical outcomes of patients residing in nursing homes and to provide supportive structured care to prevent unnecessary admissions.
