Indicators 2.2 Key Service Outcomes Tertiary specialised services play an important role in relieving pressure on acute services and facilitating discharge to the community or on-going placement. Therefore all units report process data including • Response times – waiting times for assessment, admission etc. • Length of stay • Discharge destination Key service outcome: Patients (and their family / carers) are supported to achieve their maximum potential for physical, cognitive, social and psychological function, participation in society and quality of living. Key measurable outcomes: The relevant outcome measures may vary from individual to individual, but services are required to record one or more of the following key standardised measures which are collated and reported through the UKROC database: • Functional gain measured by change in the UK Functional Assessment measure Domain 1 Preventing people from dying prematurely Domain 2 Enhancing quality of life for people with long- term conditions ✓ Domain 3 Helping people to recover from episodes of ill- health or following injury ✓ Domain 4 Ensuring people have a positive experience of care ✓ Domain 5 Treating and caring for people in safe environment and protecting them from avoidable harm ✓ (FIM+FAM) – a global assessment of functional independence reflecting both physical and psychosocial function. Reduced requirement for on-going care and care costs in the community , measured by the Northwick Park Dependency Scale and Care Needs Assessment) • Attainment of individual goals for rehabilitation measured by Goal Attainment Scaling (GAS) Services are also required to collect a measure of patient satisfaction. A range of adapted tools has been developed to facilitate feedback from patients with cognitive / communicative impairments. Other optional measures include measures of cognitive/behavioural function, quality of life tools, community integration and/or workability, as appropriate to the type of programme and intervention.

Acorns service addresses the following quality domains derived from this definition (NHS Outcomes Framework and Domains Indicators 2013/14) and is delivered in accordance with relevant NICE Quality Standards (see section 4.1) 2.2 Local defined outcomes Improved quality of life for C&YP with a life-limiting or life-threatening condition and their families (including both emotional and physical health) by: • Co-ordinated assessment and care through managed care pathways and strong relationships with other care providers • Enabling improved quality of life and symptom control for C&YP with a life-limiting or life- threatening condition • Supporting care closer to home and choice in place of death through nursing and end of life care provision. • Provision of short breaks (respite care) for C&YP (with a life-limiting or life-threatening condition) to support the emotional and physical health of carers and siblings. • Provision of pre and post bereavement support for parents/carers. • Provision of bespoke pre and post bereavement support for wider family members such as siblings and grandparents • Provision of culturally sensitive services for the diverse community groups served. Domain 2 To minimise morbidity by providing the most appropriate care for children with allergic disease. To work with secondary care network providers to ensure high quality allergy services at secondary care level. Domain 5 To ensure there is sufficient, skilled and competent multi-disciplinary workforce to manage children with allergic disease. To ensure that children with allergic disease are treated in line with national guidelines and agreed local pathways. 2.1.2

This will be achieved by procuring county-wide provision of near patient testing anticoagulant monitoring (the ‘gold standard’ of care).

The CCG has a number of clinical priorities which relates to the care for our patient population in nursing homes within the Ipswich and East Suffolk area as follows: To improve the health of those most in need To improve access to mental health To improve outcomes for patients with diabetes to above national averages To improve care for frail elderly individuals To allow patients to die with dignity and compassion and choose their place of death The remit of this contract is to improve the clinical outcomes of patients residing in nursing homes and to provide supportive structured care to prevent unnecessary admissions. The expected local outcomes from the introduction and implementation of the contract are also outlined in the service specification as: Reduction in health inequalities for residents in a Care Home environment Improvement in the falls and pressure ulcer prevention in homes Improve Long Term Condition management for residents in Care Homes Improve quality and safety of prescribing in care homes Allow care home residents to die with dignity and compassion and choose their place of death Improve residents‟ experience of care
