Member Files Clause Samples
Member Files. 12.01 The only recognized personnel file of a Member shall be maintained in the Human Resources Directorate, accessible only by persons in the Human Resources Directorate. The file shall be available and open to the Member for inspection in the presence of a Human Resources representative by contacting the Human Resources Directorate and arranging an appointment. The appointment will be arranged within three (3) days of the receipt of the request or within practical limitations.
12.02 A Member shall be entitled, upon request, to copies of any materials which they would normally have a copy. Requests for copies of other documents shall not be unreasonably denied.
12.03 In the event a Member is ill, or otherwise unavailable to review their file, they may authorize, in writing, access to their file by the Bargaining Unit President, or designate, who shall be supplied, upon request, to copies of any materials which they would normally have a copy. Requests for copies of other documents shall not be unreasonably denied.
12.04 A Member shall have the right to be accompanied by a Bargaining Unit representative when reviewing their personnel file.
Member Files. Should you choose to sign up for the SDL Consulting Firm’s services, you will enter into a separate written agreement with the SDL Consulting Firm. Each file we create in conjunction with this separate written agreement is called a “Member File.” Your Member File will continue to be subject to the agreements otherwise governing them, except where so noted in the related agreement. Each Member File will be subject to the following: The terms or instructions appearing on a screen when using the service; SDL Consulting Firm’s policies and procedures applicable to the Member File and the online service; Applicable state and federal laws and regulations.
Member Files. Except with respect to matters involving transitional assistance or other matters for which WellPoint has responsibility hereunder, maintaining and updating Member and prospective Member files, and sending daily updates regarding existing Members to WellPoint in a mutually agreed upon format for purposes of claims processing.
Member Files. Subject to the provisions of this Agreement, the contents and use of the member files are for decisions respecting terms and conditions of the employment of the Member. The purpose of this Article is to ensure that the files are used in an appropriate manner and fairly represent the Member's employment history. Member files may include up to all of the following: the Personnel file, the Appointment Committee file, the Peer Review Committee file, the Human Resources file and the Payroll and Benefits file.
