Major Nonconformance. A major nonconformance is a nonconformance that adversely impacts (or has the potential to impact) mission, safety of personnel and/or equipment, performance (quality), schedule (delivery), or cost. This category of nonconformance increases risk to Moderate or High in a standard 5x5 Risk Assessment matrix. The CO will communicate a major nonconformance to the Contractor on a Corrective Action Request (CAR) form with a suspense date for the Contractor’s corrective action plan
