Maintenance Reports. To enable MOTOR CARRIER to fulfill its obligations under 49 C.F.R. Part 396 of the DOT regulations to monitor the inspection, maintenance, and repair of motor vehicle equipment operated under its authority, Contractor agrees to keep and maintain, or cause to be kept and maintained, systematic records of repair and maintenance of the Equipment; to provide MOTOR CARRIER with quarterly Equipment maintenance reports and, promptly upon MOTOR CARRIER’s request, any other inspection, repair, or maintenance records. The quarterly reports shall specify all maintenance and repairs performed on the vehicle(s), and shall be supported by paid receipts. The reports are to be submitted to MOTOR CARRIER by the fifteenth (15th) day of each month for the preceding quarter. The reports shall be submitted by Contractor on the appropriate forms of MOTOR CARRIER.
Maintenance Reports. The awarded bidder(s) must maintain records of all work performed at each station and complete a “Lift Station Maintenance Report” during each visit. The reports must be kept current by the bidder(s) at all times and must include, at minimum, the following information: 1). Station Number, 2). Station Location, 3). Service Date, 4). Detailed Services Performed, 5). ET Readings, 6). Inspection Checklist, and 7).
Maintenance Reports. 1). To enable CARRIER to fulfill its obligations under D.O.T. regulations to monitor the inspection, maintenance and repair of equipment operated under its authority, INDEPENDENT CONTRACTOR agrees to provide CARRIER with monthly vehicle maintenance reports on each unit of leased equipment provided to CARRIER hereunder not later than the 15th day of the month for previous month. The reports shall specify all maintenance and repairs performed on the vehicle and shall be supported by paid receipts
2). INDEPENDENT CONTRACTOR must submit a Vehicle Inspection Report prior to completion of this Lease Agreement. CARRIER shall have the right to remove any unit of leased equipment from service when unsafe conditions are found by D.O.T. inspection or otherwise.
3). INDEPENDENT CONTRACTORS not submitting the Quarterly Vehicle Inspection Report will be fined for non-compliance. All drivers who get “Out Of Service” will be charged with $300.00 for the first time being OOS, $500.00 for the second time being OOS, $1,000.00 for the third time being OOS, and immediate termination of employment. At any time CARRIER reserves the right to make changes to above policies at their discretion. Any non Out of Service violation as a result of roadside inspections will be fined $250.00 up to $1,000.00 per inspection. Any further major violations and negligence by the drivers will result in more serious disciplinary actions like suspension and termination.
Maintenance Reports. The Maintenance Reports shall be completed every three (3) months or every quarter, for a total of four (4) Maintenance Reports per annual performance period. The Contractor shall prepare and submit a maintenance report describing the work performed since the previous work, work currently underway, work anticipated, and an updated project schedule showing each task, milestone event and submittal required by this PWS where applicable. Within each report, the Contractor shall specify, the semi-annual and annual requirements, in addition to the quarterly requirements that are conducted during the specific maintenance report. The report shall state whether current work is on schedule. If the work is not on schedule, the Contractor shall state what actions are anticipated in order to get back on schedule. In addition, the quarterly report shall include, safety issues identified since the previous report, any customer comments and ratings, data analysis (e.g. scheduled maintenance funds spent, and top 10 recurring maintenance with frequency of maintenance included) and backup data as required. The Contractor shall also denote the frequency of the scheduled maintenance (e.g. quarterly, semi-annual, annual). The report shall reference the applicable UFC or host nation requirement to ensure that all requirement scheduled maintenance was completed.

The Authority shall throughout the Service Agreement Term keep such maintenance and repair reports as are customarily kept by Persons of established reputation operating similar properties and as a subsequent operation of the Water System would require.
