LIENHOLDER INFORMATION. The Undersigned Holder of this Agreement Authorizes the following Lender to: 1. Receive any refund for credit to my account in the event this Agreement is properly cancelled; or to, 2. Cancel this Agreement in the event that I default on my Obligation to such Lender.
