Leave Request. 1. The employee will provide notice to the University that includes the anticipated dates of absence and the type of leave the employee wishes to use during the absence. When the request for leave is for medical reasons, the employee will also provide a doctor’s statement to the University.
Leave Request. 7.7.1 Employees are required to complete “Request for Short Leave of Absence Forms”, Exhibits E(1) and E(2) and secure permission from the Division prior to taking any such leave, except in emergency situations.

Leave Request. Each leave request form shall include a requirement that the employee notify the Employer in writing of a specific time that the employee intends to return. If the employee fails to return on the date of such notice, the employee shall irrevocably and conclusively be deemed to have voluntarily quit.

Any employee who has a claim for compensation while absent because 31 of injury or illness incurred as prescribed herein, shall file a claim on the standard leave 32 form provided by the Board with his/her principal or other immediate supervisor by the 33 end of the fifth working day following the employee’s return to duty after the leave for 34 illness or injury-in-line-of-duty.
Leave Request. Employees called into service must provide the District with 30 days written advance notice of impending service or as much notice as is reasonable under the circumstances. Exceptions will be made where such notice is impossible, unreasonable or precluded by military necessity.

Leave Request. A leave of absence without pay shall be granted to tenured teachers at their request for child bearing, for rearing of children up to age 6, or for caring of unmarried children who are mentally or physically incapable of caring for themselves thus requiring custodial care. The period of time, not to exceed one year, shall be specified by the teacher, provided, however, that the year shall be extended as necessary to meet the requirements of Section 9.4. In no case shall the leave extend beyond the beginning of the second school year. This provision shall not preclude the possibility of said leave being extended to non-tenured teachers, provided, however, that said leave does not extend beyond the limits of the teachers’ individual contracts. Except in the case of emergency, the teacher shall give thirty (30) days’ notice prior to beginning leave.
Leave Request. In connection with the election to take child care leave, the teacher shall submit a request for such leave in writing. Such request shall include an estimated commencement date and return date. The estimated commencement date shall be the physician's projected date the teacher will no longer be disabled from teaching due to childbirth or pregnancy-related disability; or, for the non-birthing parent, the date in which the child is expected to be in the care of said parent; or, in the case of an adoption, the agency estimated date when the child will be turned over to the parent.
Leave Request. All leaves of absence shall be requested from the Employer in writing as far in advance as possible stating the amount of time requested. A written reply will be given by the Employer in response to the request. Leaves of absence for the purpose of extending vacation shall be entirely at the convenience of the Employer. A written reply to grant or deny the request shall be given by the Employer within thirty (30) days. Temporary employees shall not be eligible for any leave of absence.
