Lead Agency Role. Lead Agency will play the following role in the Coalition: Provide ongoing strategic input on priority programing based on community metrics. Serve as a voting member of the Steering Committee. Oversee initial coalition building logistics to include the following: Creation, collection, and storage of Coalition agreements for general membership. Convening and oversight of the initial steering and sub-committee participation including initial appointment of Steering Committee members and creation of participation documents. Provide the initial draft of Coalition guidelines including but not limited to codes of conduct, media interaction policies, etc., for modification and approval of the Steering Committee. Setting of initial Coalition meetings, agendas, tracking of minutes, etc., until the roles of the Steering Committee members are defined and accepted.
Lead Agency Role. The MCO is the lead agency responsible for conducting required face to face assessments and reassessments and support planning services for its Enrollees to determine the need for: (i) An institutional level of care (see contract sections 4.22 and 6.7.13) pursuant to Minnesota Statutes § 256B.0911, subd. 4, paragraph (a); (ii) HCBS waiver service eligibility (see contract section 6.1.16) including level of care determination for individuals who need an institutional level of care as defined pursuant to Minnesota Statutes §§ 144.0724, subd. 11, or 256B.092; (iii) Service eligibility including state plan home care services (see e.g., contract sections 6.1.18 and 6.1.39) identified in Minnesota Statutes § 256B.0625 subd. 6, 7, and 19 paragraphs (a) and (c); (iv) Determinations must be based on assessment and support plan development with appropriate referrals.
