Late Policy. Any client who is late for a lesson, will only be taught for the remainder of the agreed upon lesson time frame. After 15 minutes, the swim instructor is not required to fulfill the lesson. It will be considered a “no show” and the client will be charged for the lesson. Any swim instructor who is late for a lesson should add on the missed amount of time to the end of the lesson. If this is not possible and the swim instructor is over 15 minutes late, there will be no charge for the lesson. CANCELLATION POLICY: There will be no charge for lessons canceled with at least a 24-hour notice. Any cancellation less than 24 hours will be considered a “late cancel” and the client will be charged for the lesson. As a courtesy, we will take into consideration illness or last-minute emergencies. You may purchase swim lessons at the front desk using cash, check, credit card, or credit card on file. Please tell the staff that you want to purchase swim lessons under your name, not the child if the lesson is for a child as well as the name of your instructor. Any unused, pre-paid lessons will expire one year from the purchase date.

Should you wish for your child to have access to these medications, you must sign a waiver and leave a supply with the school’s administrative assistant or nurse aide. The medication to be administered by designated school personnel must be sent directly from the pharmacy or physician’s office or brought to school by the student’s parent/guardian. The school must receive a Medication Administration Directions Form signed by the student’s physician and parent/guardian. The following information must be printed clearly on the original medication container: ● Student’s Name ● Name of the medication ● Dosage ● Time the medication must be taken Bring in only the amount of medication that is needed for a school day. In the case of prolonged need, send in the amount for a clearly specified period such as one week or one month. Extra medication will not be sent home with a student. All medication will be kept in a secure location in the school.
Late Policy. Our office is one of the few physical therapy practices that allows 50-55 min treatments for each of our patients one-on-one with a therapist. Because our practice is a specialty practice, we schedule appointments for 50-55 minutes each visit. That time is dedicated just for you. We do not see any additional patients during that time like traditional Physical Therapy. Because we offer one on one treatments for that 50-55 minute, your insurance carrier will only pay for time that you are in our office receiving treatment or being assessed. Therefore, any time you miss for that treatment will be your responsibility. Insurance does not cover late or missed appointment fees. Insurance is billed in increments of 15 minutes therefore any missed treatment time for 8-15 minutes will cost you approximately $25.00 per 15 minutes (depending on your insurance carrier). This does not include your copay or coinsurance. We want every patient to receive as much treatment as possible each visit. So, it is very important to arrive for your appointment on time and be prepared to stay for approximately 50- 55 minutes. Late Policy. If you are running late for a reservation, we ask that you notify our staff by calling 424- 425-7500 as soon as possible so we may hold your spot. If you arrive late for an appointment, you will be charged for the full duration of that appointment. If you are more than 10 minutes late for your reservation and have not contacted us by phone, we have the right to give the entire duration of your reservation to another child, and you will forfeit the full amount.

Late Policy. Please arrive to each scheduled session on-time. If you’re running more than 5 minutes late, please notify me. I will wait up to 15 minutes for late arrivals. Please acknowledge that any delays to the start of a scheduled appointment will not be a cause to extend provided services beyond the remainder of the scheduled time.

Late Policy. There is a Boarding Fee (at current rate) per dog if the guest is picked up after closing time on departure day. All guests must have been treated with an effective form of tick & flea control treatment. If external parasites are discovered upon check-in, guests will be immediately dismissed or treated (by Wagging Lounge discretion) with an effective form of flea/parasite control at owner/guardian’s expense and additional private room charges may apply at the owner/guardian’s expense. All guests must enter and exit the facility with a leash, collar or harness, and an identification tag. All dogs must be leashed and in control by their owner while in the facility. Dogs may react differently in the reception area due to leash reactivity, protecting a parent, or other reasons and should be kept away from other dogs and Parents during the check in/out process. Please note that some of the dogs in the reception area may be there for the first time due to Evaluation. All guests staying at WAGGING LOUNGE DOG RESORT must be at least 16 weeks old. To join daycare, all guests must be spayed or neutered (unless under 7 months of age). Once they are over 7 months of age they must be spayed or neutered in order to socialize with the other guests in our daycare. Dogs in Heat: Female dogs in heat can stress our other guests and are not accepted. Any dog going into heat during their stay will become private, and will need to be picked up immediately. Additional private room and care charges may apply at the owner/guardian’s expense. To ensure the safety of all other guests and employees, all Daycare/Boarding guests must undergo a temperament Evaluation before taking part in social activities. No Boarding or Daycare reservations may be made until the completion and passing of an Evaluation.
