Jails a. The Contractor shall ensure: i. Screening and assessment services to individuals who are in jail and are suspected to have a serious mental illness; and ii. Continuity of care, discharge planning and timely sharing of information for enrolled persons with a serious mental illness who are in or are leaving the jail.
