Information Technology System for the Care Management Program. The MCP’s information technology system for the Care Management Program must maximize the opportunity for communication between the MCP, the PCP, the member, and other service providers and care managers. The MCP must have an integrated database that allows MCP staff who may be contacted by a member in care management to have immediate access to, and review of, the most recent information with the MCP’s information systems relevant to the case. The integrated database may include the following: administrative data, call center communications, service authorizations, care treatment plans, health assessments, care management notes, and PCP notes. The information technology system must also have the capability to share relevant information with the member, the PCP, and other service providers and care managers. The goal is to integrate information from a variety of sources in an effort to facilitate care management needs for the member.
