Individually Identifiable Sample Clauses

The "Individually Identifiable" clause defines what constitutes information that can be linked to a specific individual, typically in the context of privacy or data protection. This clause clarifies that data such as names, addresses, identification numbers, or any other details that can directly or indirectly identify a person are considered individually identifiable. By establishing this definition, the clause ensures that parties understand which types of information are subject to privacy obligations, thereby helping to protect personal data and comply with relevant laws or regulations.
Individually Identifiable. Health Information is health information, including demographic information collected from an individual, that: (i) Is created or received by a health care provider, health plan, employer, or health care clearinghouse; (ii) Relates to the past, present, or future physical or mental health or condition of an individual; the provision of health care to an individual; or the past, present, or future payment for the provision of health care to an individual; and (iii) Either identifies the individual or with respect to which there is a reasonable basis to believe the information can be used to identify the individual.
Individually Identifiable. Health Information that is transmitted by electronic media; (b) maintained in any medium constituting electronic media; or (c) transmitted or maintained in any other form or medium. “PHI” shall not include education records covered by the Family Educational Right and Privacy Act, as amended, 20 U.S.C. § 1232g, or records described in 20U.S.C. § 1232g(a)(4)(B)(iv).
