Common use of Individual Request Clause in Contracts

Individual Request. If an Individual makes a request for an amendment directly to Business Associate, Business Associate will within 10 business days forward such request in writing to Covered Entity. Covered Entity will be responsible for making all determinations regarding amendments to PHI and Business Associate will make no such determinations.
