Immunization Administration. Administration of any medically necessary vaccines subject to a case-by-case review by Primary Provider. Member will be responsible for subsequent cost of the actual vaccine. We do not have vaccines on hand in the office and can only be administered in office when the vaccines can be obtained on a case by case basis.

CHD’s may also use internal electronic medical report (EMR) documents to report visits.
